RN Home Health Jethro Health System

Jethro Health is looking for an experienced Home Health Agency RN to provide care for patients in the North Shore area including Lynn, Peabody, Nahant, Revere, Winthrop, East Boston, Chelsea, Salem, Swampscott, Wakefield, Reading, Woburn, Saugus, Revere.

The role is part?time with potential to become full?time. Candidates should submit an updated resume to the recruiter.

Responsibilities

• Conduct thorough assessments of patients' health status and needs in their home environment.
• Develop and implement individualized care plans based on patient assessments and medical history.
• Provide education and support to patients and their families regarding health conditions, medications, and self?care techniques.
• Assist with discharge planning from hospitals or rehabilitation facilities, ensuring a smooth transition back to home care.
• Monitor patient progress and adjust care plans as necessary to meet evolving health needs.
• Collaborate with interdisciplinary teams, including physicians, social workers, and therapists, to coordinate comprehensive care.
• Document all patient interactions accurately in compliance with ICD?10 coding standards.
• Offer health coaching to empower patients in managing chronic conditions effectively.
• Provide specialized services such as esthetic laser treatment when applicable.
• Support family planning initiatives and educate patients on related health topics.

Skills

• Strong clinical skills with a focus on acute care and senior care practices.
• Excellent communication skills for effective patient education and collaboration with healthcare teams.
• Proficient in discharge planning processes and the ability to navigate complex healthcare systems.
• Knowledge of ICD?10 coding for accurate documentation of patient encounters.
• Experience working in a teaching hospital environment is preferred but not required.
• Ability to provide compassionate care while maintaining professional boundaries.
• Strong organizational skills to manage multiple patient cases effectively.
• Commitment to ongoing professional development and staying current with best practices in nursing care.

Pay

$38.00 - $45.00 per visit
